Marijuana Moment"},"content":{"rendered":"<p>A top federal health agency is seeking to promote studies into the efficacy of a variety of harm reduction policies\u2014including decriminalization and safe consumption sites\u2014as part of a push to combat the overdose epidemic.<\/p>\n<p>While the Biden administration has yet to take a position on policy proposals to authorize safe consumption facilities, the National Institutes of Health (NIH) put out a pair of requests for applications (RFAs) on Wednesday for an effort that will provide funding for efforts to investigate how that and other harm reduction policies could help address the drug crisis.<\/p>\n<p>Specifically, NIH wants to <a href=\"https:\/\/grants.nih.gov\/grants\/guide\/rfa-files\/RFA-DA-22-046.html\" target=\"_blank\" rel=\"noopener\">establish<\/a> a Harm Reduction Network that seeks to \u201cincrease our understanding of the effectiveness, implementation and impact of existing and new harm reduction practices to address the ongoing opioid crisis and substance use disorder more broadly.\u201d<\/p>\n<blockquote class=\"twitter-tweet\" data-width=\"550\" data-dnt=\"true\">\n<p lang=\"en\" dir=\"ltr\">HEAL Initiative: Harm Reduction Policies, Practices, and Modes of Delivery for Persons with Substance Use Disorders (R01 Clinical Trial Optional) <a href=\"https:\/\/t.co\/59VOnbTVmj\">https:\/\/t.co\/59VOnbTVmj<\/a><\/p>\n<p>\u2014 NIH Funding (@NIHFunding) <a href=\"https:\/\/twitter.com\/NIHFunding\/status\/1476219594972942339?ref_src=twsrc%5Etfw\">December 29, 2021<\/a><\/p>\n<\/blockquote>\n<p>A parallel <a href=\"https:\/\/grants.nih.gov\/grants\/guide\/rfa-files\/RFA-DA-22-042.html\" target=\"_blank\" rel=\"noopener\">RFA<\/a> calls for a coordination center within the network to provide \u201clogistical and coordination support,\u201d \u201cdata harmonization and data sharing supports\u201d and \u201cresearch and clinical practice resources.\u201d<\/p>\n<p>Applications are being accepted for projects that involve \u201c(1) developing and testing new harm reduction strategies; (2) examining how to effectively implement new and existing harm reduction strategies; (3) expanding the settings and delivery models through which harm reduction strategies are deployed;\u202fand\u202f(4) examining the impact of new harm reduction policies implemented at state and local levels.\u201d<\/p>\n<p>\u201cHarm reduction services\u202faim\u202fto\u202fprevent\u202for minimize\u202fadverse\u202foutcomes related to substance use,\u202fsuch as\u202ffatal\u202fand\u202fnon-fatal\u202foverdose\u202fand\u202finfectious\u202fdisease transmission,\u201d the notice states. \u201cExamples of established harm reduction\u202fapproaches include\u202fnaloxone,\u202ffentanyl test strips (FTS),\u202fsafer smoking equipment, and sterile syringes, as well as\u202fhuman immunodeficiency virus (HIV) and Hepatitis C virus\u202ftesting.\u201d<\/p>\n<p>The notices from NIH and component agencies such as the National Institute on Drug Abuse (NIDA) say that \u201cemerging\u201d harm reduction policies \u201cinclude decriminalization of various drugs, police and prosecutor-driven\u202fdeflection and diversion efforts, and\u202fthe\u202fauthorization of safe\u202fconsumption\u202fsites.\u201d<\/p>\n<p>They also note that the White House Office of National Drug Control Policy (ONDCP) is prioritizing harm reduction policy as a means of preventing overdoses.<\/p>\n<blockquote class=\"twitter-tweet\" data-width=\"550\" data-dnt=\"true\">\n<p lang=\"en\" dir=\"ltr\">HEAL Initiative: Harm Reduction Policies, Practices, and Modes of Delivery for Persons with Substance Use Disorders: Coordination Center (R24 Clinical Trial Optional) <a href=\"https:\/\/t.co\/KPWFRhszv9\">https:\/\/t.co\/KPWFRhszv9<\/a><\/p>\n<p>\u2014 NIH Funding (@NIHFunding) <a href=\"https:\/\/twitter.com\/NIHFunding\/status\/1476219600421359622?ref_src=twsrc%5Etfw\">December 29, 2021<\/a><\/p>\n<\/blockquote>\n<p>NIDA Director Nora Volkow has repeatedly expressed concerns about the harms caused by the criminalization of drug possession, and has specifically called out the racial disparities in enforcement as a major problem\u2014including in an <a href=\"https:\/\/www.marijuanamoment.net\/top-federal-drug-official-discusses-rise-in-psychedelics-use-and-the-need-to-study-marijuana-from-dispensaries\/\" target=\"_blank\" rel=\"noopener\">interview with Marijuana Moment<\/a> and in <a href=\"https:\/\/www.marijuanamoment.net\/top-federal-drug-official-says-we-dont-need-more-research-to-show-criminalizations-racist-impact\/\" target=\"_blank\" rel=\"noopener\">several<\/a> <a href=\"https:\/\/www.marijuanamoment.net\/top-federal-drug-official-says-criminalization-creates-stigma-and-harms-health-of-consumers\/\" target=\"_blank\" rel=\"noopener\">op-eds<\/a>.<\/p>\n<p>When it comes to safe consumption sites, Volkow said earlier this year that she is open to continuing to explore \u201chow these support systems as a community can help people, for example, engage in treatment, how they can prevent them from getting infected from HIV and how they can <a href=\"https:\/\/www.marijuanamoment.net\/top-federal-drug-official-admits-legalizers-were-right-about-teen-marijuana-use-and-touts-psychedelics-therapeutic-potential\/\" target=\"_blank\" rel=\"noopener\">prevent them from overdosing and dying<\/a>.\u201d<\/p>\n<p>New York City launched the first sanctioned safe consumption site late last this month, and advocates have been left wondering how the federal government will respond given its role in blocking a Philadelphia non-profit, Safehouse, for launching its own harm reduction center.<\/p>\n<p>New York City officials say the sites\u2014where people can use currently illicit drugs in a medically supervised environment where they have access to treatment resources\u2014<a href=\"https:\/\/www.marijuanamoment.net\/uss-first-safe-drug-consumption-sites-are-already-saving-lives-by-stopping-dozens-of-overdoses-in-less-than-a-month-nyc-officials-say\/\" target=\"_blank\" rel=\"noopener\">have already saved dozens of lives<\/a>.<\/p>\n<p>That\u2019s just one part of NIH\u2019s research initiative on harm reduction, however. Here\u2019s a description of the research topics it wants to explore:<\/p>\n<ul role=\"list\">\n<li class=\"Normal\" role=\"listitem\" data-aria-level=\"1\" data-aria-posinset=\"1\" data-font=\"Symbol\" data-leveltext=\"?\" data-listid=\"21\">\n<p class=\"Normal\">Research to develop and test novel approaches and\/or settings for harm reduction service provision,\u202fincluding strategies that involve sectors outside of the healthcare system\u202fand strategies that are not contingent upon face-to-face interaction<\/p>\n<\/li>\n<\/ul>\n<ul role=\"list\">\n<li class=\"Normal\" role=\"listitem\" data-aria-level=\"1\" data-aria-posinset=\"1\" data-font=\"Symbol\" data-leveltext=\"?\" data-listid=\"21\">\n<p class=\"Normal\">Research that seeks to understand individual- and system-level barriers to providing effective, scalable,\u202fand sustainable harm reduction services, such as individuals forgetting or being unwilling to carry naloxone or fentanyl test strips, workforce shortages, funding limitations, and stigmatizing attitudes toward individuals with SUD.<\/p>\n<\/li>\n<li class=\"Normal\" role=\"listitem\" data-aria-level=\"1\" data-aria-posinset=\"2\" data-font=\"Symbol\" data-leveltext=\"?\" data-listid=\"21\">\n<p class=\"Normal\">Research to develop and\/or test strategies to\u202faddress\u202fidentified\u202fbarriers to effective, scalable,\u202fand sustainable harm reduction\u202fservices,<\/p>\n<\/li>\n<li class=\"Normal\" role=\"listitem\" data-aria-level=\"1\" data-aria-posinset=\"3\" data-font=\"Symbol\" data-leveltext=\"?\" data-listid=\"21\">\n<p class=\"Normal\">Research on strategies to ensure that individuals from vulnerable, understudied, and\/or hard-to-reach populations have access to and are benefitting from harm reduction services.<\/p>\n<\/li>\n<li class=\"Normal\" role=\"listitem\" data-aria-level=\"1\" data-aria-posinset=\"4\" data-font=\"Symbol\" data-leveltext=\"?\" data-listid=\"21\">\n<p class=\"Normal\">Research on the implications of\u202femerging harm reduction policies, including their effectiveness in decreasing adverse outcomes and\u202fthe\u202fbarriers\/facilitators to\u202fsuccessful\u202fimplementation\u202fin real-world settings<\/p>\n<\/li>\n<li class=\"Normal\" role=\"listitem\" data-aria-level=\"1\" data-aria-posinset=\"4\" data-font=\"Symbol\" data-leveltext=\"?\" data-listid=\"21\">\n<p class=\"Normal\">Research on harm reduction strategies\u202ffor\u202findividuals using methamphetamine and other stimulants<\/p>\n<\/li>\n<\/ul>\n<p>Nine applicants will be selected to conduct the studies as part of the five-year program. It\u2019s approving up to $6.75 million for the projects in fiscal year 2022.<\/p>\n<p>The new notice also talks about applications for marijuana research, stressing that any such studies are \u201crequired to measure and report results using a <a href=\"https:\/\/www.marijuanamoment.net\/feds-announce-new-standard-thc-dose-to-be-used-in-marijuana-research-effective-immediately\/\" target=\"_blank\" rel=\"noopener\">standard delta-9-THC unit<\/a> in all applicable human subjects\u2019 research.\u201d<\/p>\n<p>\u201cThe goal is to increase the comparability across cannabis research studies. A standard delta-9-THC unit is defined as any formulation of cannabis plant material or extract that contains 5 milligrams of delta-9-THC,\u201d it says. \u201cA justification should be provided for human research that does not propose to use the standard unit.\u201d<\/p>\n<p>With respect to safe consumption sites, activists in several cities have attempted to establish these centers in recent years.<\/p>\n<p>In October, the Supreme Court&nbsp;<a href=\"https:\/\/www.marijuanamoment.net\/supreme-court-wont-hear-case-on-legalizing-safe-drug-consumption-sites-but-activists-are-undeterred\/\" target=\"_blank\" rel=\"noopener\">rejected a request to hear a case<\/a> on the legality of establishing the facilities in Philadelphia, but the case is still before a lower court and advocates are eagerly waiting for a DOJ response to show where the agency decides to come down on the issue under the Biden administration.<\/p>\n<p>Under a mutual agreement between federal officials and Safehouse, the deadline for the administration to submit their position was pushed back on until March 7. It had previously been extended to November 5 of this year. Advocates see that as a positive sign.<\/p>\n<p>The White House drug czar, Rahul Gupta, recently said that it\u2019s&nbsp;critical to explore \u201cany and every option\u201d to reduce overdose deaths, and that&nbsp;<a href=\"https:\/\/www.marijuanamoment.net\/white-house-drug-czar-open-to-safe-consumption-sites-for-illegal-substances-amid-overdose-crisis\/\" target=\"_blank\" rel=\"noopener\">could include allowing safe consumption sites<\/a>&nbsp;for illegal substances if the evidence supports their efficacy.<\/p>\n<p>The ONDCP director previously said that he couldn\u2019t talk about the harm reduction centers due to the ongoing litigation related to Safehouse, but he seemed more open to the possibility in the recent interview with CNN.<\/p>\n<p>The secretary of the U.S. Department of Health and Human Services (HHS), Xavier Bacerra, also recently signaled that the Biden administration would not move to block the establishment safe injection sites, stressing that \u201cwe are literally trying to give users a lifeline.\u201d<\/p>\n<p>But a department spokesperson&nbsp;<a href=\"https:\/\/www.marijuanamoment.net\/biden-administration-wont-block-safe-consumption-facilities-for-drugs-top-federal-health-official-says\/\" target=\"_blank\" rel=\"noopener\">later walked those remarks back<\/a>, stating that \u201cHHS does not have a position on supervised consumption sites\u201d and the \u201cissue is a matter of ongoing litigation.\u201d In any case, it would be up to DOJ to decide whether to pursue operators of the facilities under the Controlled Substances Act.<\/p>\n<p>Bacerra was among eight top state law enforcement officials who&nbsp;<a href=\"https:\/\/oag.ca.gov\/news\/press-releases\/attorney-general-becerra-joins-multistate-amicus-brief-support-public-health\" target=\"_blank\" rel=\"noopener\">filed<\/a>&nbsp;an amicus brief&nbsp;<a href=\"https:\/\/www.marijuanamoment.net\/bidens-health-secretary-pick-could-help-reclassify-marijuana\/\" target=\"_blank\" rel=\"noopener\">in support of the Safehouse\u2019s safe injection site plan<\/a>&nbsp;when he served as California\u2019s attorney general.<\/p>\n<p>The Biden administration has generally promoted the concept of harm reduction as part of its drug policy, but it hasn\u2019t formally weighed in on safe consumption sites in particular.<\/p>\n<p>Advocates have put the current situation in no uncertain terms. They say the harm reduction centers could mean the difference between life and death for countless Americans who consume currently illegal drugs.<\/p>\n<p>The early data out of New York City signals that the facilities could prevent significant more deaths than the health department projected. Its feasibility study found the safe consumption sites could save as many as 130 lives per year.<\/p>\n<p>The legal complication for these harm reduction sites is principally related to a so-called federal \u201ccrack house statute\u201d that makes it a felony to use a location for the manufacturing, distribution or consumption of controlled substances.<\/p>\n<p>A coalition of 80 current and former prosecutors and law enforcement officials\u2014including one who is Biden\u2019s pick for U.S. attorney of Massachusetts\u2014previously filed a brief urging the Supreme Court to&nbsp;<a href=\"https:\/\/www.marijuanamoment.net\/80-top-law-enforcement-officials-including-a-biden-nominee-urge-scotus-to-hear-safe-injection-drug-case\/\" target=\"_blank\" rel=\"noopener\">take up Safehouse\u2019s safe consumption case<\/a>.<\/p>\n<p>While New York City is the first to open the harm reduction centers, the governor of Rhode Island did sign a historic bill in July to&nbsp;<a href=\"https:\/\/www.marijuanamoment.net\/rhode-island-governor-signs-historic-bill-legalizing-safe-consumption-sites-for-illicit-drugs\/\" target=\"_blank\" rel=\"noopener\">establish a safe consumption site pilot program<\/a>.<\/p>\n<p>Massachusetts lawmakers&nbsp;<a href=\"https:\/\/www.marijuanamoment.net\/massachusetts-lawmakers-approve-bill-allowing-safe-injection-facilities-for-illegal-drugs\/\" target=\"_blank\" rel=\"noopener\">advanced similar legislation last year<\/a>, but it was not ultimately enacted.<\/p>\n<p>A similar harm reduction bill in California, sponsored by Sen.
